What it is & how to Register

Patients with a Medicare card who have attended two or more in-person GP appointments at Werribee Medical Centre within the past two years may be eligible to register with our clinic for MyMedicare.
MyMedicare is a free and voluntary program that supports ongoing care by strengthening the relationship between patients and their chosen general practice. Registering with one regular clinic and GP allows your doctor to better understand your medical history and coordinate your care over time, helping to support improved long-term health outcomes.

Benefits of registering with MyMedicare

By registering, the benefits to you include
– More personalised and continuous care through a formal connection with your GP
– Medicare rebates for longer telehealth consultations with your GP
– Better coordination of care if you attend hospital frequently
– Development of chronic disease management plans and mental health care plans, if eligible

Important to know
– MyMedicare does not provide discounted appointment fees or subsidised consultations
– Choosing not to register will not affect the quality of care that you receive from your healthcare providers

* From 1 November 2025, mental health care plans must be completed by the clinic a patient is registered with under MyMedicare.
