Chronic Health Management

Chronic disease is any health condition that has been, or is likely to be, present for the long term. These include diabetes, heart disease, asthma, arthritis, depression and anxiety, neurological and other conditions. We recognise that living with a chronic condition can be extremely stressful and can involve some significant life changes.  We offer well-managed and planned health care management with regular reviews.

Women's Health

As women’s bodies change throughout their life so too do their health care needs. We are well versed in women’s individual needs across the different phases of life and pride ourselves on providing culturally sensitive best practice treatment.

Mental Health

MyMedicare

Patients with a Medicare card who have attended two or more in-person GP appointments at Werribee Medical Centre within the past two years may be eligible to register with our clinic for MyMedicare.
MyMedicare is a free and voluntary program that supports ongoing care by strengthening the relationship between patients and their chosen general practice. Registering with one regular clinic and GP allows your doctor to better understand your medical history and coordinate your care over time, helping to support improved long-term health outcomes.

Benefits of registering with MyMedicare

By registering, the benefits to you include
– More personalised and continuous care through a formal connection with your GP
– Medicare rebates for longer telehealth consultations with your GP
– Better coordination of care if you attend hospital frequently
– Development of chronic disease management plans and mental health care plans, if eligible

Important to know
– MyMedicare does not provide discounted appointment fees or subsidised consultations
– Choosing not to register will not affect the quality of care that you receive from your healthcare providers
